1. How much does this plan cost?
2010-2011 YFS Health & Dental Plan Fee
Full-Time Student Fee:
$211.97: is charged per individual student for 2010-2011 Benefit year, please note that this is the total cost for 12 months of coverage. This is NOT a monthly rate.
$197.64: To add ONE dependant (note: this is in addition to $211.97)
$293.67: To add more than One (note: this is in addition to $211.97)
Part-Time Student Fee:
$544.91: is charged per individual part-time student.
Couple: $530.71 (note: this is in addition to 544.91)
Family: $793.67 (note: this is in addition to $544.91)
Please note that each dependant gets the same amount of coverage such as same services and products.
Definition of dependants:
Children: biological, adopted/legal guardian under 24 and in school full-time. Proof required, e.g. Birth Certificate, etc.
Spouse: includes common-law and same sex spouse.
Proof: official documentation with both names (common); for example. A lease dated one or more years prior to current benefit year 2010-2011 etc.
Other proof that is acceptable can be a marriage certificate, Notice of Assessment (NOA) from Revenue Canada for the previous year/s, for example 2009, 2008 etc.
2. Why is this plan mandatory?
When the health plan was first created, students voted for a mandatory system. The health plan is mandatory instead because otherwise the cost would be too great and the benefit too little. In an opt-out package, insurers are able to spread out the potential risk (i.e. the likelihood that they would have to pay the claim) amongst a large group of people, driving down the cost for everyone.
3. Who is eligible for the YFS Health and Dental coverage?
Full-Time Students: All undergraduate students (with the exception of Osgoode students) who are taking 15 credits or more from fall and winter semesters are eligible for the plan.
Part-Time Students: Please enroll yourself and your dependants manually at the YFS Health and Dental Plan office, which is located in room 106 of the Student Centre. Please see "Adding Dependants/Family" for rates
Students with Dis/Ability: Registered full-time (minimum 12 credits) and recognized by the Office with Disabilities as disabled, need to enroll manually at the YFS Health and Dental Plan office, which is located in room 106 of the Student Centre.
Graduating Students: If you currently have less than 15 credits, and have applied to graduate, please provide us with a transcript that verifies you have completed over 80 credits. Please come to enroll manually at the YFS Health and Dental Plan office, which is located in room 106 of the Student Centre.
The YFS Health and Dental Plan fee (levy or premium) will be automatically charged to your York University Student Account Statement ANNUALLY during academic year. This will happen either the 18 of August, 18 of September or the 18 of October (for 10/11 Fall & Winter terms). For the winter term only, the charges will appear on your student account either the 18 of December, 18 of January or the 18 of February (for 2011 winter term only). Please check the status of York University Student Account Statement online on the 18 of each month). It is charged once per year. For example, $211.97 for 2010/2011 benefit year.

4. How can I add my family members onto my coverage?

5. Opt-out Online only:
How can I cancel the plan?
Important: The opt-out deadline is Friday, October 15, 2010
IF YOU HAVE COMPARABLE COVERAGE from your parents or work/other, you can opt out starting August 16, 2010 until the 15 of October 2010. Winter term online opt-out starts on January 3, 2011 until February 25, 2011 (for 2011 winter term) on-line at https://www.heath.ca/unv07/optout.asp?Step=0
After completing the online opt out, you will receive an e-mail confirmation including an application number.
If you do not get a confirmation e-mail, it means it did not go through or was not successful. If you did not receive your opt-out email, please take a look in your spam folder of your email account that you used when completing the online opt-out form.
Keep that e-mail and wait until the November 18 statement or the 18 of each subsequent month. The plan fee will be reimbursed to your student account. If you have already paid the YFS Health & Dental Plan levy, you will receive a refund; if you have not yet paid the YFS Health & Dental Plan levy; the amount will be reversed into your York University Student Account Statement. 6. How often do I need to opt out of YFS Health & Dental Plan?
You need to opt-out of YFS Health & Dental Plan ONCE A YEAR because the plan fee charges annually.
7. Making the Claims:
Please submit all your original receipts with a claim form for reimbursement at our office (106 Student Centre); Keele Campus. Glendon College Students- please submit all original receipts at Student Affairs at Glendon Campus at York Hall C114. During the off school year, you can mail all the claims to our office anytime. See the below contact information for mailing address. We receive claims/log and process them daily; finally we courier them by federal express via (FedEx) twice per week.
8. When does YFS Health & Dental Plan benefit year start and end?
YFS Health & Dental Plan benefit year always starts in every September 1 and ends in August 31; for example September 1, 2010 to August 31, 2011.
9. When do we get the YFS Health and Dental card?
The YFS Health and Dental Plan is now cardless.
There is no physical YFS Health and Dental card.

NOTE: difference between chiropodist/podiatrist (foot care professionals)
A chiropodist is a specialist in treating minor ailments of the feet, including fitting patients for Orthotics. Chiropodists are not graduate physicians and their activity should be restricted entirely to the treatment of corns, ingrown toenails and other minor foot conditions. They can only do soft tissue surgery under local anesthesia. They can prescribe only topical creams and some antibiotics. According to their licensing, the difference between a Chiropodist and a Podiatrist is that a podiatrist in addition to soft tissue surgery, can do bone surgery (on the forefoot) and x-rays also. OHIP does cover some expenses toward a podiatrist in Ontario.
11. Where can I use my coverage?
You can use anywhere, anyplace or anytime in Canada. However, in order to get additional dental discount, Green Shield Canada, in partnership with the National Student Health Network, has created a national program including over 300 dentists from coast to coast, ‘Student Dental Discount Network'. Participating dental providers have agreed to provide students with a discount. To find participating dentists visit:

12. When does the plan get activated and how can I get my money back?
There is always black out period for a month or so to process all the opt-outs in the beginning of benefit year.
The pay direct "card less" for prescription drugs and dental coverage and the booklet including the plan information will be available and activated with the all providers in November 1, 2010. You can get discount automatically with your policy number
(YFS + Student Number +00).
In the interim, you need to pay the full amount for all services and products. Please submit all your original receipts with a claim form for reimbursement at our office (106 Student Centre); Keele Campus. Glendon College Students- please submit all original receipts at Student Affairs at Glendon Campus at York Hall C114. During the off school year, you can mail all the claims to our office anytime. Therefore, you can get the cheque back from Green Shield Canada (benefit provider) after the plan is activated on November 1, 2010.
A cheque will be mailed to the address you wrote on your claim form or you can register now for "Subscriber Online Services" and experience instant access: for example, Direct Deposit etc: Register today at https://onlineservices.greenshield.ca/SubscriberOnlineServices/AccessMgmt/Public/AccountRegistration.aspx . Once registered, a password for direct deposit will be mailed to the address Green Shield Canada has on file for you. Just register once and you will be connected anywhere and anytime.
13. Is there a time limit in which I can submit my claim?
Yes. You must submit your claim within 90 days after benefit year; for example, (September 1, 2010 to August 31. 2011) Deadline to submit claim would be November 2011).
