Claims Form:

For dental and prescription drugs, you will get a discount automatically with your policy number. 

Apart from dental and prescription drugs, you are required to pay for the service and submit the Claims Form and receipts to the YFS Health and Dental Office.  A cheque will be mailed to you within two weeks to the address listed on your claims form.

Plan members can also subscribe to a Direct Deposit service available through Green Shield at https://onlineservices.greenshield.ca/SubscriberOnlineServices/AccessMgmt/Public/AccountRegistration.aspx.
